Not long after Jimmy Casket resurfaces, Johnny Ghost is sent to a mental institution called Saint Jones Psychiatric Center in Idaho. As his sanity slowly deteriorates, Jimmy's power grows stronger. Despite the best efforts of the doctors, nurses, and Toast, Johnny Ghost is chained in a solitary room within his mind and Jimmy takes full control and murders everyone in the facility while toying with Toast as long as he can.
